RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT BY AGREEING WITH THE TERMS OF THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE
Participation in the sport of pickleball and use of any recreational facilities involves a risk of accidental injury or death.
I assume all risks and hazards incidental to the game, and I, my heirs, executors, administrators release from responsibility and agree to indemnify and hold harmless The Corporation of the District of North Cowichan, the Skinny Singles Shootout Tournament organizers and their sponsors, directors and volunteers, Vancouver Island Pickleball Association, their officers, directors, and volunteers, the group and the facility where we play, their officers, directors, independent contractors, volunteers and all employees from any and all liability, including negligence, for any loss, illness, damage, expense or injury, including death, that I may suffer.
Photo/Video Release Waiver
I hereby consent to and authorize the use of photographs/videos/names of participants by the Vancouver Island Pickleball Association in a variety of publications (e.g. our website, newsletters, etc.) to help promote the sport of pickleball.
I hereby certify that I am 19 years of age or older, I have read fully the previous paragraphs and I know, understand and appreciate these and other risks that are inherent in the activity. I hereby assert that my participation is voluntary and that I knowingly assume all such risks for myself and for any underage participant under my guardianship.
Registering for this tournament reflects your agreement to the terms of this document, knowing you will waive certain legal rights including the right to sue.
